Probation Check-In Form (Formulario de Registro de Libertad Condicional)

Probationers instructed to report for appointments online should do so by filling out this form on the monthly date assigned by their probation officer.

Pretrial Check-In Form (Formulario de Registro de Servicios Previos al Juicio)

This form is for those who are not on probation but are complying with pretrial services.

Travel Permit Application (Solicitud de Permiso de Viaje)

Some probationers may be permitted to travel outside the state of Indiana with permission of the court. Consult your probation officer at least 10 business days before making travel plans, and be prepared to provide at a minimum the information in the form provided here. (Note: if you have any conviction or adjudication for a sex offense as defined by Interstate Compact rules you may be subject to registration in the location in which you are traveling.)

Use this form to express interest in Where Does Your Money Go?, a two-part financial education workshop held at the 4-H Building. Dates and times for the workshop will be scheduled once sufficient interest is received. Completing this form does not guarantee enrollment but helps us plan future sessions.
Probationers who complete this class may be eligible for a 30-day early release.
